If pre-paying for the full 7-week period ...

you can pay by cash or cheque. Please make cheques payable to SMES Home and School. If, during the 7-week period, students wish to change their pizza orders, simply let the teacher know on Monday when weekly orders are taken.

If pre-paying for more than one child at the school ...

you may submit one payment for all of your children. Simply attach your cash or cheque to this form and return it to one of your children's teachers. If submitting a cheque, please write the names of all of your children on the memo line.

Once-a-Month Friday Lunch (without Milk)

A Sub and Nachos Lunch will be available on Friday, November 13/09 for $2.50 each. Friday lunches can be paid in cash on the preceding Monday or added to your pre-payment.

Forms (and pre-payments) are due Thursday, October 29th, 2009. Please fill out the form on the back and indicate which, if any, lunch and milk programs you'd like your child(ren) to receive.

***** The grade 3/4 and 5/6 students who were in the last lunch program and went on the class trip on Sept. 24^th have a $2.00 credit towards this terms lunch program. Please deduct the $2.00 from your payment amount.
